At Stream Services, we specialise in delivering Supported Independent Living services (SIL) for NDIS Participants in remote, country and outback communities across Queensland.
Our mission is to create home and living services in your community.

OUR SIL HOMES
We currently offer a four-bedroom SIL shared home in Clermont as well as provide in-home SIL services for Participants in Innisfail and Longreach.
We are extremely excited about opening new SIL homes to meet Participants’ needs right across regional, rural and remote Queensland, so please get in touch to discuss your requirements!

Just because there isn’t a SIL home in your town right now, doesn’t mean that Stream Services can’t move quickly to create one — as we have done successfully for other Participants.

YOUR OWN HOME?
Great news! We also offer Supported Independent Living services to Participants living in their own homes.
Our amazing Lifestyle Support Workers will be there by your side as you work towards your goals and achieve new levels of independence, both at home and out in the community.
Need Specialist Disability Accommodation?
We can work with external SDA providers for Specialist Disability Accommodation (SDA) that is purpose-built for your needs, including accommodation with customisations including ceiling hoists, height-adjustable benchtops and wheelchair accessibility.

A SIL home is far more than accommodation that fits your physical needs. It’s a place where you feel safe, secure, and supported. Our team will get to know you so we can match you with the right house, the right care model, and the right people, from your team of friendly Support Workers to those all-important flatmates!
When we’ve secured your spot in a SIL home and got all the paperwork out of the way, we’re here to make the move to your new home absolutely seamless. This is a really exciting time, as you settle into your new bedroom, make friends with your housemates, and get into the daily rhythm of life with your support team. There’s so much to look forward to, from activities at home with your ‘roomies’ to getting out in the community.
We are always here to support you. The Stream Services team are your cheerleaders as you take this step into independent living. And we stay by your side, checking in regularly to check that you are happy in your home and have everything you need.

We present a range of care models to you to choose from, and are happy to trial different arrangements until we land on the one that suits you best.
We also think that matching the right support worker to each Participant is a bit of an art form — one that Participants and their families say we are extremely good at!

Our SIL services include daily living assistance, nursing care, and activities in the community.

Stream Services also offer a range of Allied Health services for Participants, including Psychology, Occupational Therapy and Dietetics. We also have an excellent and growing team of Lifestyle Support Workers and Support Coordinators that really understand the challenges and opportunities of delivering top-tier NDIS care in remote and regional Australia.
Connecting with Community
Community is everything in Australia’s regional and country areas, and connecting our SIL Participants with their local community is at the core of what we do.
We pride ourselves on embedding Participants into the town they live in, through a rich program of activities, sports, volunteering…and just getting out there!
The local community is the most powerful tool we have for making our Participants feel they belong.
Case Study: Jeffrey’s Story
We were contacted about a Participant in regional Queensland who had been stuck in hospital due to major barriers in setting up Supported Independent Living (SIL) services – especially building a full team in a remote community.
A large NDIS provider asked us to assist with staffing and later transitioned him to Stream Services when it became clear we could deliver complex services where others struggle.
Working with his family, hospital, GP, Allied Health, the Support Coordinator, and NDIA, we created an innovative model for rapid discharge – bringing in temporary staff while recruiting locally and designing a structure tailored to his needs.
He has now been thriving for over a year in the first ever SIL home in Longreach.
