Our Services

Stream Services is the brain child of NDIS Registered Provider Clermont Country Care. CCC was founded after a cry for help from a remote country GP, who could not find services for her patients who had disabilities. Lead Coach and Founder, Alec McConnell, learned from Dr Sarah McLay (Doctor and owner of Clermont Country Practice) about the unique challenges that people in remote communities face in accessing services which are taken for granted in the cities. He could see that there was a huge gap in NDIS services for people who were living in small, isolated communities across Queensland and Australia.


We focus on aiding individuals and their support networks to develop multi-faceted caring environments. These environments are purposefully designed to foster rich healing and learning opportunities now and into the future. Through a number of treatments, interventions and educational practices we can help manage mental health issues such as depression, anxiety and stress, identify and work through barriers for change and development or provide clinical interventions for other mental health challenges.

Exercise Physiology

Accredited Exercise Physiologists are allied health professionals, providing exercise and lifestyle therapies for the prevention and management of chronic disease, injury and disability.


Our dieticians provide evidence-based guidance related to your gut health, diet and nutrition through an individualised plan for your specific health concerns. Our dieticians work across a range of clinical settings such as the community, medical centres, private hospitals and aged care facilities. With this comes a range of expertise allowing us to provide the service you require.

We work with people living with permanent and significant disability to assist them to achieve participation in everyday occupations to lead healthy and independent lives. Occupational therapists are frequently funded by the NDIS to work with a participant to achieve their goals.


Our Support Coordinators are experts at helping you understand your plan, activate it, and find service providers and products best able to meet your individual needs.


Assistance with, or supervision of, personal tasks of daily living.  Support delivered by a TTP provider (including COVID19 loading).
