Our Story

SUPPORT FOR LIFE IN YOUR LOCAL COMMUNITY

In 2019, a country GP in Clermont reached out for help. Dr Sarah McLay couldn’t find services for her patients with disabilities, and the only option was to send them away to the cities. That didn’t sit right with us.

Stream Services, originally Clermont Country Care, was founded to change that. We set out to deliver NDIS supports in the communities where people actually live, so they could stay close to the people they love, in the the towns they know.

We are here to fill the gaps. Stream Services exists to offer people living in regional, rural, and isolated parts of Australia access to the NDIS services often taken for granted in the cities.

OUR MISSION

Our passion is to deliver exceptional supports and services that work for the Participant, even if that means starting from scratch to design unique routines, learning modules, and activities that are truly life-supporting

We are agile and compassionate, and wherever possible, we find a way to say ‘yes’. Because we are part of rural communities ourselves, we understand deeply the challenges and joys that come from living in the country, and just how important community is to all our lives. Our mission is to help overcome isolation and service gaps for our NDIS Participants and help them reach their goals.

THE STORY BEHIND OUR
FIRST SIL HOME

In 2019, we opened the first ever SIL home in Clermont. Since then, we’ve established SIL services in Longreach and Innisfail, each one tailored to the community and the people we support.

But we’re not limited to these locations. If someone needs support in their community, we work with them to make it happen. We can set up services for one person in one town. Every situation is different, and we build solutions around what each Participant actually needs.
