Stream Services delivers Supported Independent Living (SIL) and other NDIS services in regional and remote Queensland, so you can live close to your family and community again.

OUR SIL HOMES
Right now, we have a shared home in Clermont with four bedrooms. We also support people in their own homes in Longreach and Innisfail.
We’d love to set up more SIL homes across Queensland. If you live somewhere we haven’t been yet, get in touch and we’ll see what we can do.

A SIL home is more than a place to sleep. It’s where you feel safe. Where you feel at home. Where the people around you know you and care about you.
We take our time matching you with the right house, the right team, and the right housemates. We’ll try different things until we find what works.
And we stay close. We check in with you regularly, to make sure you’re happy.

You get to choose how you want to be cared for.
We’ll show you a few different ways we can support you, and you get to choose what works for you.
Matching the right support worker to you is one of the things we do best. It’s a bit of an art form, and the families we work with tell us we’re good at it.

The care we give
Our SIL services includes:
- Help around the house: cooking, cleaning, the day-to-day things.
- Nursing care when you need it.
- Getting out in the community: shopping, sport, friends, activities.
- Support with your health appointments.
- Help building skills and working towards your goals.
We also have a team of Allied Health Professionals, including Psychology, Occupational Therapy, and Dietetics, who can support you as part of your plan.
Your community is everything
Your friends, your family, your local footy club, the shop on the corner - that's what home is.
We'll support you to play an active part in your community.
Sport, volunteering, hobbies, just getting out and saying hi to people.
The more you're part of your community, the more you feel at home.
Jeffrey’s Story
Jeffrey is from Longreach. He wanted to live there, in his own place, with the right support around him. But with no SIL home in town, he couldn’t leave hospital. The supports he needed just weren’t there.
So we worked with Jeffrey, his family, and his support team to set one up. We found a local team. We put good people around him. We made sure his home felt like his home.
Jeffrey has been living well in Longreach for over a year now. His is the first SIL home the town has ever had.
