We operate across multiple locations in Central Queensland with an ever-growing team of experienced NDIS Lifestyle Support Workers, Support Coordinators and Allied Health professionals.
At Stream Services, we specialise in delivering Supported Independent Living services for NDIS Participants in remote, country and outback communities across Queensland.
CORE SUPPORTS
Support Coordination
Support coordination is designed to assist you in optimising your available supports within your plan.
This service falls under the category of capacity building support, aimed at helping you comprehend and effectively utilise your NDIS plan to work towards your desired goals.
Our Support Coordinators are experts at helping you understand your plan, activate it, and find service providers and products best able to meet your individual needs.
Community Access
Whether your interests include dancing, sports, or a beachside lunch, our Lifestyle Support Workers are eager to support you in living your dreams. We can assist you in various ways, such as:
- Connecting with your community through your hobbies and interests
- Transportation to and from events
- Social support in the community
- Social skill development
- Assistance with goal setting
Assistance in Daily Living
Guidance in honing everyday task skills can pave the way for greater independence.

Personal Care
When you require additional support for your daily living tasks, we’re here to lend a helping hand.
- Mobility and hoist transfers

Dietetics
Our dietician Sue provides evidence-based guidance related to gut health, diet and nutrition through an individualised plan for specific health concerns.
Also working across a range of clinical settings such as the community, medical centres, private hospitals and aged care facilities, Sue brings a range of expertise which supports providing the tailored service you require.
Occupational Therapy
Liz is our Occupational Therapist and works with people living with permanent and significant disabilities. She works collaboratively with Participants to support participation in everyday occupations and live healthy and independent lives.

ALLIED HEALTH
Stream Services offers a range of Allied Health services for Participants, including Occupational Therapy and Dietetics.
We have an excellent and growing team of Lifestyle Support Workers and Support Coordinators that really understand the challenges and opportunities of delivering top-tier NDIS care in remote and regional Australia.
Care Tailored to You
It’s YOUR choice how you want to be cared for. We present a range of care models to you to choose from, and are happy to trial different arrangements until we land on the one that suits you best. We also think that matching the right support worker to each Participant is a bit of an art form — one that Participants and their families say we are extremely good at!

Independent Living, Local Connections
Our Supported Independent Living (SIL) services are designed to help Participants live confidently and achieve their goals close to home.
We focus on building everyday skills, encouraging community involvement and supporting each person to live the way they choose – safely, independently and surrounded by familiar faces.
Case Study: Jeffrey’s Story
We were contacted about a Participant in regional Queensland who had been stuck in hospital due to major barriers in setting up Supported Independent Living (SIL) services – especially building a full team in a remote community.
A large NDIS provider asked us to assist with staffing and later transitioned him to Stream Services when it became clear we could deliver complex services where others struggle.
Working with his family, hospital, GP, Allied Health, the Support Coordinator, and NDIA, we created an innovative model for rapid discharge – bringing in temporary staff while recruiting locally and designing a structure tailored to his needs.
He has now been thriving for over a year in the first ever SIL home in Longreach.
