Got to Esthers place had a yarn to Woof about walking Jess early in the mornings instead of the middle of the day when it is the hottest. He was happy for me to do that if Alce was happy. Turned on the sprinkler took Jess for a walk through on the dirt tracks. It did not take long before she was pulling up at the shade on the road and wanting to drink out of the puddles on the road. I would not let her drink out of them as I would not let my dogs drink it. Got back to Esthers place some how Jess got next door. Got her back and put her inside. Turned off the sprinkler. At Participant Esther’s request, the scheduled meeting went ahead via phone (Esther is currently in Brisbane) with Support Coordinator Cherie and Manager Alec. Esther is requesting an urgent plan review as soon as possible as her circumstances have changed as baby Hugo is growing in weight. An increase in funding would allow Esther to purchase a new wheelchair and a raised cot and have access to increased physiotherapy which Esther requires.

Esther requested that a support worker roster be sent to her each week, which has not been happening as usual recently. This was referred to the Lifestyle Support Worker, Montana, wh0 is also completing the rostering at present.

Manager Alec suggested to Esther that a plan review request be referred to Chris Hickman (NDIA Planner – Remote Planning Team – Mackay) due to the urgency and the change of circumstances. Chris was contacted and linked into the phone call with Esther. Following Esther’s consent, Support Coordinator Cherie will forward via email relevant documentation provided by Esther and a copy of the Functional Capacity Assessment recently completed by Occupational Therapist, Liz Doyle, to Chris Hickman, in preparation for a plan review. Support Coordinator will advise Esther of outcome, once Chris has reviewed the documentation.

Support Coordinator Cherie will follow up with Occupational Therapist, Liz, via email regarding obtaining measurements required for a new wheelchair for Esther.

Met with Esther today and updated her risk assessment, Home Safety Checklist and gathered information for her storyboard (service plan). We also discussed progression with the OT and what Esther was still waiting for so I contacted Liz straight after the meeting to get the ball rolling. Also had a discussion with Cherie on how we could help Esther more. Esther talked about her new needs now that Hugo is in her life and she is a Mum and how we can help her with her new role. 3 hours were spent visiting Esther to gather information, completing risk assessments and liaising with Liz and Cherie to ensure that Esther’s needs are being met.

Note- Fly-In-Fly-Out supports: Fly in costs for specialist staff visiting remote communities are charged to NDIS Plans at cost-only which is different for each visit. Stream Services charges only the exact direct costs of travel related to the trip to the remote community. The total amount is divided by the number of hours the specialist spends with everyone during the visit. For this trip the total costs will be divided by 15 hours spent delivering services which will be approximately $49.00 for travel time and $177.80 for travel expenses such as flights and accommodation. This visit has been used to update the service plan (Storyboard), Home Safety Checklist and Risk assessments which are requirements of NDIS reporting and auditing.

SC Cherie met with participant Esther H at her residence at 13:00 today. Esther stated that her plan which ended on 04/06/2022 had been rolled over for a further 12 months and to date Esther has not been sent a copy of the current NDIS Plan. Esther has requested a plan review due to a significant change in her circumstances and need for increased hours for a support worker to support her, both within her home and with transportation to scheduled appointments with baby Hugo. These appointments are in Clermont, Emerald and Brisbane. Esther also stated that Hugo has weekly appointments with a dietician in Clermont and monthly appointments with a paediatrician in Emerald.

Esther provided SC Cherie with documentation which includes a list of upcoming medical appointments, Connected Care Program Health Care Summary and for baby Hugo, and a letter of support from Clinical Nurse, Connected Care Program Children’s Health Queensland Hospital and Health Service. SC Cherie photographed these documents for uploading to Participant folder and returned the original copies to Esther. Esther stated that baby Hugo also has further scheduled surgeries in September and December 2022 and January 2023.

Esther has consented to Liz (OT) contacting her regarding new wheelchair measurements and a functional capacity assessment. Esther stated that he has had spinal surgery in the past, and the whole right side of her body is weak, and she has decreased muscle tone. Esther also stated that there are issues with her being unable to push Hugo in a pram, whilst using a cane in her right hand. Presently she carries Hugo in a front harness, however this is “not great” for her spine. This will become a further issue when she is supported by her assistance dog, Jess, eventually for public access. Esther is keen to explore options for managing a baby whilst using a cane and with support for her assistance dog.

Esther stated that lifting baby Hugo is now causing nerve damage in her arm (as in “tennis elbow”) which she has been advised may only be managed by steroid injections or surgery. Esther stated she is having a scan this Thursday in Emerald to ascertain the level of damage.

Esther stated that she currently has a support worker daily for 2 hours a day (except for weekends when her husband is at home), however, would benefit from increased support from a support worker to care for baby Hugo and allow her to rest, which brings some relief for her spinal issues.

Esther stated that she is hoping her new NDIS plan will have increased provision for physiotherapy.

Esther stated that her assistance dog, Jess, has been receiving training from Positive Response Training via Telehealth, but this has been sporadic up to date, due to COVID restrictions. Esther stated that a pre-assessment has been completed for her support dog, Jess, but there is further training required to “polish” Jess for public access (which may take a maximum of three months to complete). Support dog, Jess, then must undergo a public access test, which would take place in Brisbane. Esther stated that Jess also requires grooming on a regular basis. SC Cherie attempted to call Zuzanna Napieralski (Community Engagement Manager) at My Plan Manager with Esther’s consent to enquire as to provisions under her NDIS Plan for the above requirements for her support dog, however, Zuzanna was not available to speak with and a message was left to contact SC Cherie.

Esther also requested support to enable Positive Response Training and a cleaner (which Esther has engaged) to send their invoices directly to My Plan Manager for payment, rather than Esther facilitating this as she is presently doing, and assistance with Service Agreements for these providers, as she has not signed a Service Agreement with them up to date.

Esther stated she will require a workplace assessment, as she is intending to return to work in January 2023.

Esther enquired as to whether Liz (OT) may have any recommendations regarding carrying Hugo, using a cane in her right hand and having her support dog, Jess, attached to her in some way. Esther requested to email any relevant documentation to SC Cherie and email address was provided.

SC Cherie to email these notes to Liz (OT) and Alec (Manager/Owner) on completion for review.

After a session of this I decided to mow the front lawn and down one side. After the storm subsided the heat returned and I called it a day.
